I did everything right, upgrades, loyalties etc.
Everybody survived except Jack, and it was right at the last second before the whole base blew up.
She dies at random it seems
Need help with alll teammates surviving at the end

She may have a low "hold the line" score. I've never had her die there though. Its been written that all characters have a "hold the line" score that relates to surviving the door guard... Mordin gets thrashed there alot, perhaps Jack isn't so good either?

Strange...was she part of your squad or part of the team that was left behind to defend the door? if the later I would suggest taking her with you or sending her back with the rescued crew members. I think what might have happened is that since Jack isn't much of a defensive character she wasn't able to hold out against the incoming hordes of collectors

Where exactly did she die? Please describe what portion of the mission, and where she was.
*If* she was loyal, the only way she really could have died is if you used her for a mission specialist she wasn't qualified for (fire team leader or tech specialist), or if you left her to "Hold the Line", and didnt leave enough Tank Types like Grunt, Zaeed, and Garrus back with them.

If she's not loyal, do not take her with you to fight the Reaper. Leave her to hold the line, and make sure that you have enough tank-types there as well, like Grunt, Zaeed, Garrus.
Do not send a tank-type squad mate to escort the crew back. Send a loyal squishy, like Mordin or Tali.

If you have a save right before you enter Omega 4, you could try talking to Jack again and smooth things over with her. Otherwise, take Tali and Legion with you to the final fight.
