I've managed to add followers well enough, but I've encountered a problem when one engages in dialog with them. Is there a generic solution to this problem? Optimally I'd assume a stage would be "generated" whenever speaking to them in any location, but I don't know if this is even possible.

You can create a stage with no area attached to it and use it for follower conversations. Make sure you check in the "use at location" option in your conversation file though.
Best exemple : take a look at Alistair's convo in the main campaign and open the stages associated with them, you'll see the setup quickly.

I'd like to add a question here.
Let's say I have a conversation and an optional follower. If he's in the party during the conversation, he has a line to say. If not, the stage should not teleport him at all. Is this possible with only one conversation file?
Edit: Never mind - found it. ^^ I feel relieved a bit.

It's a simple condition check on that particular line's plot&scripting tab. You can open dialogue file of Alistair or Morrigan for example and check the section dealing with kissing them in public.
