But should I settle for her Grimoire without killing her or just kill her - and is that even possible?

Morrigan wants me to kill Flemeth, and since we´re "a couple"
I have little choice in the matter.
But should I settle for her Grimoire without killing her or just kill her - and is that even possible?

Well, you should be able to kill her, if not wait a level or two. You could settle for the grimoire without killing her, if that is ok for you, read the dialoge ;-)

You can either kill Flemeth, or take her deal and the Grimoire and lie to Morrigan about it. Or just not do it at all.
Though if you are looking towards what Level you should befor the fight, I'd say at least level 14. I didn't have to use many potions, and Alistair and myself dps'ed and tanked her while Zevran or Leliana goes from backstabs or ranged attacks. Wynne is by the cottage of course healing and buffing us.

If you got some fire resistance and a salve or two you can try killing her while staying ranged, just make sure 1 ranged character gets the agro (the one with the most fire resistance), spread out the rest of the group an kill her.
Depends a bit on the difficulty but on normal with a decent group she shouldn't be a problem at around level 12, even if you have your tank walk up to here and engage in close combat.

I guess it depends how you wanna roleplay it. It really doesn't matter. Morrigan will believe anything you tell her.

She might not believe you RP-wise, but she doesn't let on either way. I haven't finished the game yet, just at the Landsmeet, so I don't know if she ever meets Flemeth again. However, haha, up until now, she hasn't said anything to indicate that she knew I let Flemeth go. I am hoping for a show-down though. Fingers crossed!

I killed her the first time. But I don't like to anymore since she did save your life.

I always kill her. Shes a dragon and that makes it worth killing, best not to wait for her to find you at a later date.
As for what effects it has at the end *big spoiler* after Morigan has left you it makes mention that with Flemeth dead there is no way of tracking Morigan down. So I don't know if that is possibly going to change if you let Flemeth live. I imagine though she may make an appearance in the expansion.

Does killing her provide any nice items?

All loot is just key to chest that she can give u anyway. Still in my opinion u cant kill her:
1/ her soul can travel from body to body
2/ as she mentioned its not first time when some1 came kill her
so there is very high chance she already has some backdoor - like some pet to which she can transfer if some1 succes to defeat her body and where she can retake powers to posses better body.

The loot you get is extremely poweful robes for Morrigan by using the key to open her chest in her hut.
Anyway, if you love her, surely you would want to help her to not be possesed by an ancient abomination.
