Erm,

-spoilers-

No, he dies at the end. But it's a pretty awesome scene so if you can, keep him alive for it.

You also miss out on some XPs in that the 'boss' fight for the area won't trigger if he's dead.

Never had a problem keeping him alive but then I play on easy. So maybe turn the difficulty down if you want to get him to the end. If it's just the end that's the issue, then no, he doesn't survive, sorry. (I always feel bad for just leaving him lying there - it would've been better if you could have at least reported it to the Chantry so they could go and fetch him. Ah well).

Just take Wynne to heal him, or Morrigan, if she has healing. Force field also helps.

Force Field is your friend here. Otto draws a lot of aggro from the demons and he's not very good at defending himself. When you are facing the more difficult fights simply drop a force field on him and then clean up as the Demons persist in going after Otto.

You can't complete the quest unless he is alive, so he can die!

Also, I FECKING LOVE THIS GUY. I looooove him. He is such an interesting character. Why did you kill him Bioware? Why? :( You made me sad. Is there a way to force him to stay??

I just did the part with Ser Otto today for the first time. He was really awesome. I loved his concept, his look, etc. It was a shame to kill him in such a helpless way.

....A pitchfork? Really? They should throw an insta-kill pitchfork at the Wardens then, that would be more beneficial to their side.

Oh well. RIP Otto, see ya on the flip-side.

I know...I must admit I saw it coming, though. When you defeat the first demon and he stands with his back to the door I thought something big sword would come straight through and kill him. (First time I played, anyway). So pitchfork through the back wasn't a massive surprise after the second demon. I still felt bad for him though, he was a great character.

In my opinion, there could be more quests like the orphanage one. It was one of the most interesting side-quests in DA:O.
And I agree, Ser Otto was awesome on so many levels it's shame he died so quickly. He deserved much better death scene than this.

I can't really feel sorry for him, honestly. I mean, yeah he was a great character, but he's all "Yah, I'm blind" so I was all, "And you can still fight?" And he was like, "No, not really." So I'm like, "Yeah, well maybe you should leave the demon hunting to the people who can, you know, see... and stab things properly?" And he still insisted on coming with.

So when the pitchfork hit him I felt kinda bad for a second, then I remembered he'd still be alive if he weren't such a stubborn ass and shrugged it off.

He refused to be brought down by his obvious disability and in fact downplayed it when you mentioned it.  That's brave and noble.  One of the reasons I liked him.

Brave and noble would be saving a little boy trapped in a well, stupid and idiotic is trying to fight demons when your old and blind.

Thats my point, you shouldn't have to babysit him and keep him alive, he should have just waited outside and not tried to be a hero.

I would second (third?) the forcefield.

I would also like to add that Ser Otto was the death I felt worst for. More than any other death in the game- including my PC's parents (I was expecting that). That is a personal thing, brought a bit due to my guilt in assuming all templars were merely tools for the Chantry. To see that feeble old blind man wanting to do something that I believed was right...well...I wish I could have saved the poor dude in the end.

He died during the fight where the demon came out of the floor and the other demons popped up all around. Is there more after that or is that the end?

If there wasn't a cutscene, then it wasn't the end. I'd rather not say more and spoil it entirely. You already know far to much to get the full effect.

This is a fairly intense cutscene, not just dialogue along the lines on the banter.

According to the journal the quest is complete but it says something about never knowing what otto planned so I thought I was missing something.

I like that he is in the game. He tries to do the right thing at all times, which is the hallmark of a good man. I find it hard to hate the templars, even playing my blood mage, because of Ser Otto.

It wasn't casual banter but it was 4am after a couple of glasses of yummy stuff Posted Image so I can't remember what he said.
