1. Does the problem happen in a specific location in the game or after a specific sequence of actions?
When Samara and Morinth are fighting. All sounds "vanish" until a few seconds after you've picked who to kill.
Also various lines get randomly cut off :
- in the Lazarus project facility, the subtitles go like "Miranda : Good work, Shepard. Coast is clear". I never got to hear Miranda saying "Coast is clear".
- in the shuttle just before Freedom's Progress, the subtitles for Jacob's line : "Good to have you here, Shepard". He gets cut off before saying "Shepard".
- during Mordin recruitment quest, the first time he says "Will be here when you come back" (or something), I never heard the last part of the sentence.
- during Garrus' loyalty mission, "Do you know what else is bad for business?", "business" gets cut off sometimes.
It happens elsewhere, but those are what I remember.

4. No, but interesting during my first two playthrough on normal, there was no problem, even when I could make a choice of who lives. Once I raised the difficulty to veteren for my third, all future playthrough had that error, regardless of difficulty.

The sound cuts out on the Samara/Morinth fight if I'm using a Female character.
Other than that, no problems.
I'm using the Xbox 360 version.

1. Samara/Morinth fight, every time the same spot, during three independent playthroughs. Then occasionally if I rapidly skipped dialogue/cutscenes by pressing "X", but that was pretty rare.

1. What everyone else said: Samara/Morinth a little bit after Morinth lands after she's been knocked against the window. First time it happened, I went back and started again from the previous auto-save (just before going into the club). Second time, saved in Morinth's apartment, went from there. Third time, just decided to keep playing.

1. - Same as the others, during the Samara/Morinth confrontation. Always cuts out when the biotic "force field" starts, always resumes when you choose who to kill. Played NTSC version, played PAL version, same result.
- Sometimes the sounds also cuts out when updating Thane on his target when you're doing his loyalty mission.
- And finally, although it oddly seems to happen with only one of my characters, the music cuts out after passing through the Omega-4 relay when the Occuluses chase the Normandy. This happens only with a character who has not updated the Normandy, it plays just fine with all the others, so I guess I have a problem with the slightly different cutscene that is played when you take a beating from the Occuluses. The music does not resume unless I reload the autosave created at the start of the battle in the Normandy's cargo hold.

It actually appears to be pretty random in mine. It can happen at the end of regular dialogue (as I'm playing), and in various cutscenes. Also, the Thane romance scene is COMPLETELY out of sync until the first dialogue choosing.

1. I never had any audio issues that I noticed on my first playthrough, but on subsequent playthroughs it seems to happen randomly all over the place throughout the entire game. The only spot I can definitely remember it happening repeatedly is Jacob's line on the way to Freedom's Progress, the last line before you land where he says something like "Good to have you here, Shepard". The "Shepard" gets cut off every time. I have subtitles on, so it makes lost words/lines pretty obvious.

For audio cut out you mean when in some occasion the game cuts the dialogues?
In this case, I experienced this problem in some occasion.
1-I don't remember exaclty where, but it happened in some situation, always with the same dialogues, example speaking with Miranda when you become more intimate, or with Joker during the last scenes, and with some random character.
Always with the same phrases.
I have the Italian version of Mass effect 2.
