Mass Effect 2 is all about forming a squad. You get to know, love and hate the lifes, attitudes and reasons of every character you recruit for your squad. And while you are busy with filling up your team the whole game, everything wich counts in the end is to bring them back home safety.
So for me, the only possible way in Mass Effect 3 would be, that you have exactly the same Squad as in Mass Effect 2, with, lets say, two or three more possible Squadmembers to recruit.
So this would mean:
If you loose all but two Squadmembers in ME2, you will have just two Squadmembers in ME3 wich is totally ok for me, and i´m sure for everyone else.
If you begin a new Mass Effect 3 Game, without an import you will have all squadmembers available, because surviving the suicidemission with everyone alive would be the intended shepard-way.
With this way of handling the squadmemberdeathproblem the developers are not in danger at creating neverseen content. Everyone who begins a new game without import sees everything, everyone with an import and a normal gameplaystyle sees everything expect one or two squadmember storys. And everyone with an ME2 import where all squadmembers died wanted it so anyway
Imo this is the best solution because the people who played Mass Effect 1 and 2 don´t have a problem with playing the game with half of their crew because they know why and how their squadmembers died.
