Second go around, non-imported, took Miranda's side. I still couldn't get the option during the fight at ~90%. Nor could I ever charm Jack even at 100% of the Paragon bar. Ditto on Zaeed, tried the Paragon route and didn't get his text (pretty high too, 80ish). Couldn't resist Morinth either (significantly lower paragon at that point though).
Now, knowing it was coming, I did Miranda's (I do Jack's early just to make her wear a shirt already) last on the second go around. Plus, I always snark TIM a bit, wasn't sweating the +2 Renegade that much from occasionally picking a lower dialog option to change things up for Saint Shepherd. Sounds like my acerbic wit cost me. I guess that prevents you from gaming the system but it does give you kind of a false impression of Blue Bar = Blue Text. Plus, it renders your Shepherd a pretty flat character.
% of the Blue Bar apparently doesn't mean that much for the fight in absolute terms. Will try import Shepherd and doing the missions earlier... Ain't going to give up needling TIM when given the option though.

should get access to the additional conversation options. FIX it or I swear I will not buy Mass Effect 3.
I have WAAAAYYYYY to many hours in this game to get screwed like this.

some opportunities. Doing the mission earlier can be slightly in your
favor only because you haven't missed as many opportunities yet.
If that's true then it's quite possibly the most retarded alignment system I've ever heard of ._.

Admittedly whenever a Paragon option came up I took it and if no Paragon option was possible (i.e. most talks with Jack), I took the Rengade option. But I think Kim Stolz's reveal of the way the system works was the key for me, because I brought over bonus Paragon points from my ME1 save due to already being a 100% Paragon in the first game.
*shrug*
I don't think that the system is broken... probably just snowballs too quickly.

1.Have an Imported ME1 character otherwise it won't work.
2.Do not do the LM of Miranda or Jack until you have the death mask.
3.Do first Jack's LM then Miranda's.
4.Your paragon must be at least at 75%.(I did it with only 1 box in the meter empty,the rest full.)

Been reading this thread and a few others talking about not being able to make the important paragon choices. I’ve been running into a bunch of these issues after reimporting my lvl 28 me2 character.
Spoilers below…
First of all, I wasn’t able to paragon convince zaeed on his loyalty mission and I had nearly three bars of paragon. First time I played through, I got it with one and a half bars. Next, I did Samara’s loyalty mission and I wasn’t given the choice to pick either her or morinth this time, even though I had similar paragon scores. Last time I was able to paragon finish the conversation and choose between them.
I’ve been saving the Jack/Miranda confrontation because I know that’s a tough one. But now I’m almost all out of secondary missions to raise my paragon, so I’ll let you know soon. I’ve heard in a few places that the paragon requirements raise with your character level, so that my be what’s causing some of the issues. I dunno… generally it seems like a bad idea to re-import your me2 character. I’ve run into much more bugs this time around…

I’ll try Zaeed’s again but I think my character being level 30 is making the paragon requirements too high. I’ll post what I find soon.

EDIT and the death mask does not help because you are not wereing it at the time of the fight

I did Miranda's quest right after she gave it to me, then as soon as I could I did Jack's and was able to paragon convince them to chill out and not lose loyalty. Did Tali's quest right away too and Legion as soon as possible and was also able to difuse the situation as a paragon.
There's just something wonky with the amount of time that lapses between going to worlds and quests if you spend too much time and leave loyalty quests for too long you can't convince your friends of anything no matter how many points you have.

edit: I see people have issues with other loyalty quests, I couldn't convince Zaeed at one point either and it was because I had put off doing his quest too. So I'm pretty sure about this theory, do not put any loyalty quests off, ever just do them as soon as you can, you can do N7 missions after the game even so don't spend too much time on those extras.

Thanks for this. Not having the Paragon option for the Miranda/Jack fight was greatly annoying.

its a good story, but this paragon system..... hate it.
dont think in playing ME3 that way.... I'll stay loyal to Ash, but I dont want let mirando or jack die on that last mission....

I am playing 2nd time around on an imported ME1 character, so L 30 now and some few side missions and loyalty ones to do Paragon points are 3/4 almost and 1/4 Renegade, but the Miranda/Jack fight options I want are grayed out. Not tried the mask yet.

I've been searching forums trying to find a solution and one (can't remember where) gave the impression that this scenario isn't really reliant on your morality as such but more on your interactions with the characters. It also implied that rather than metagame your way around this problem it is better to let the story unfold natural.
Going with this approach in mind I'm doing my second playthrough now and:
-Am playing down the paragon path.
-Recruited Jack as soon as I could.
-Kept dialouge between the characters as often as I could. The more the better works I think.
-Haven't recruited any others for the squad.
-Did Jacks loyalty and talked straight after.
-Talked as much as I could to Miranda and then did her loyalty next.
-Was given the option to difuse the catfight.
Worked out ok and my paragon level was barely past the first bar and I didn't import a ME1 game.
Also I had the deathmask equiped.

It's also usefull for unlocking content otherwise not available as well as raising your paragon /renegade and starting resources...
Such as the arachni queen envoy.
The feron colonist.
The conrad verner reunion... amongst others.
