While in my opinion "betrayal" often feels like a cheap way for a story to get emotional rise, i do think that Bioware with a good spin could pull of something epic. As for who it could be i think the only real options that stand out to me are the vermire survivor/liara and even those two are ify outside the indoctrination angle.

If i had to imagine a "twist" in ME3 id probably go with something along the lines of: liara striking it off on her own early in the game for some kind of prothean plot device (possibly as penance for her actions/behavior during ME2) to help Shepard only to fall victim to some form of reaper indoctrination and confront Shepard later prompting the player to make the obligatory choice of trying to save her/ put her out of her misery. yes its a bit hair brained and cliche but hey what do you want from a 5 minute brainstorm hehe.

additionally on the topic of udina/TIM being traitors, im not sure either would fit the definition as betrayal implies trust, something a lot players lack when dealing with either character.

I've felt for a while the most likely character conflict would be Ashley/Kaidan sent by the alliance or council to hunt you down as a rogue agent. Makes perfect sense to me, if you are renegade, you are still working for Cerberus and that creeps the Citadel out. If you are paragon the high likelihood is you are gathering an eclectic array of allies that would creep the Citadel out, Rachni, Krogan, Quarians(very likely as renegade as well) as well as any number of rogues from the terminus systems(also likely as renegade). They don't trust you and think you are pulling a Saren.

So they send the one loyal agent they have who knows Shepard well enough to have a shot at taking him down.
