The first time I played through, restarting because I lost Miranda's
loyalty after I helped Jack, I was able to convince the Synthetic
Insights lady to take on the quarian. This time through I got more
points and did more missions but I don't have the options anymore. I
don't have to options for the False Positives on the Citadel either. Is
this a glich or should I have a certain amount?
Not enough Paragon points?

Your Paragon needs to be VERY high. Almost full.

In the ME2's paragon/renegade system, you are working to retain the charm and intimidate options, not earn the options. It is the design flaw of the system.
One of the developers provided an elegantly thought-out explanation, but I chose the renegade path and simplified it to one blunt sentence. It is based on the percentage of how many point you have earned out of how many points you could earned from conversations and actions performed.
