I'm coming in pretty late on this. I'm not in favor of an across the board "Shepard has PSTD" for a couple of reasons. One is a storytelling reason. Shepard has a lot empty space as an in-game character, which I think gives the player a greater freedom to role-play him/her. They fill in the spaces of Shepard's personality and motivation based on their preferences. I can Shepard having PTSD running very much counter to their Shepard and alienating another part of the fan base.

The second reason is, as the wife of a PTSD sufferer, I have some real reservations about incorporating such a storyline well. It's difficult to explain, but many of the classic symptoms are far worse than what the description sounds like. An example, my husband describes his nightmares as full-blown flashbacks that happen while he sleeps. It's very pervasive to every part of your life. I don't know if a video game could portray that well without derailing the main storyline.

And something I don't think should be taken into consideration, but is worth stating for me, neither my husband nor I could play if there were an unavoidable PTSD mechanic. His symptoms tend to get worse when he watches anyone going through symptoms, and it's a little too close to home for me to enjoy.
