I'm hoping I'm not the only one with this (It might mean I'm crazy);
When my people are engaged with their target, they sometimes stop attacking for no reason. When I check their status, they still have the active target reticule on the enemy and I'm required to re-order the attack for them to continue.
Furthermore; they sometimes stop attacking altogether if the target moves slightly. This can be a result from a knockdown or the target itself moving a few paces away to engage someone else.
This can even happen with tactics, which sometimes leaves them taking a long time to readjust, even though from what I've read, their default action would be to continue attacking the target I designated.
Unless I've missed something?
---------------------
Another issue, sometimes the enemy get stuck on corners in their path finding attempt to reach me.

This happens to me with Sten constantly. I'll send him to attack then, seemingly at random, he'll stop attacking, run back to my side, and just stand there like a big dumb rock until I send him to attack again.
My guess is that it has something to do with how the tactics are setup. Since all of his tactics are abilities, if he has no stamina with which to activate any of these abilities, he'll just stop attacking altogether.
I still call this a bug as the characters should still auto attack regardless. Also, if Bioware is going to force us to configure tactics for all characters to make up for their crappy AI the least they could do is not limit how many tactic slots we have.

It happen with tactics turned off as well; where auto-attacks should still go off without a hitch. They're capable of activing their skills and using items, they just don't auto-attack very well. Hopefully something patchable.

Same here, everytime I reorder them to attack.

/bump My party members stop following me and they won't attack until i selected them and instruct them.
I check the tactic and make sure that they attack nearest visible enemy. However they won't attack at all.

I just recently started to have this problem. I'd send them into attack and they would stop auto attacking. So unless I set them to aggressive I end up having to manually get them to do EVERYTHING.
Can't have them aggressive all the time because I'd end up dying from all the mobs.

Same here as well.
*plot spoilers for Orzamaar below*
AI is extra broken during the Anvil of the Void (the one part of the trial with the four faces on a pedestal and you have to strike the spirit anvils to damage it) as they would just walk up to it and stand there. I would tell them to walk away and they would, but then would walk right back to the center of the platform and not do a dang thing. I ended up having to pause ever tenth of a second in order to keep issuing commands to every in order to finish it.
*end spoilers*
But it has happened through out the game in various battles where they would just stand around even though they had commands to attack. They would ignore their tactics to heal / block / taunt / every thing which made things interesting to say the least. I don't mind micromanagement when I am expecting to have to do it but when I'm sitting there wondering wtf is going on it's quite annoying.

Wondering if there's a fix to this yet, as it's very very annoying. It's more noticeable on melee characters, even when on aggressive and free to move (not Hold). I have a PC rogue and shale as melee chars in my party, and they both will sit around if a) their target dies and
their target moves out of melee range.

If you've installed the toolset you need to try the "Fix for strange issues..." in the troubleshooting FAQ sticky

This problem is directly related to behavior you have/have not chosen for your companions.
Default behavior is what is automatically assigned to your characters. With default your allies will not attack unless a condition is met for one of their tactics or if they are given orders by the player.
Change their behavior to whatever way you wish them to act and they will follow by the rules set by their behavior modified by the tactics you have set in place for them.

I find that setting their behavior works just fine. It's when you order them to hold position it gets fickle. Obviously someone ordered to hold position is not going to run and attack a 'nearest visible enemy'. They will only attack if the enemy walks by them. For mages and archers, hold position sometimes breaks their auto attacking. I think it all has something to do with whether or not your character is attacking, however. Not who, but whether. I find putting them where I want and then turning off the hold position command works great for this. If they are ranged, they will just whip out a bow or staff and start blasting away.

I'm playing with all tactics disabled from the start and I have this problem. It is as Endoque says: Auto-attacking stops when either: (a) current target dies or (
current target moves out of attack range. (a) is not so of a problem and it seems logical if you play with no tactics at all. However (
is frustrating. And I'm sure I don't confuse (
with other reasons which stop auto attack as getting stunned etc.
