I am not sure if it is a bug or if it was intended but when I played a New Game+ I was never given any prompt for a choice for two particular choice in the game:

1) Chosing between Samara/Mornith during Samara's loyalty quest

2) The Advanced Weapon Training on the Collector ship

In both cases I was stuck with the same choices I had made from my previous completed game.

Is a player stuck with these choice on their first playthrough permanently?

For Samara/Morinth you need an almost full parangon/renegade bar to be able to make a choice, if not, you help Samara. Regarding the collector ship yes, you must stick with what you have already chosen.
