After the conversation following Jacob's Loyalty mission, did anyone get the vibe that the two of them used to be involved? I haven't played Mass Effect Galaxy, but I know the two of them played a role in it. Could their conversation be referencing that?

Their relationship happens after Galaxy and before ME2. Though Galaxy ends with the big fragging clue they will be an item. So it is assume they get together after. ME2 starts and you learn it got close and then grew apart fast.
He even says so before his loyalty mission. I think you can get the dialogue as soon as a few in, even before Horizan. You can ask if he and Miranda were together (if a male Shepard keep pressing him if you tell him you are interested in her).

You can even ask him about it, and he'll say that there was something, but he's all standoff-ish. Not too fond that his commander is asking him personal questions and sh*t.
