In the Faq I read that eqipment and items outside of those from DLC will transfer over.  I asume then that those items contained in the party storage chest at warden's keep must be removed in order to carry over?

Yes, it says whatever is in your inventory. Therefore your personal inventory, not chest inventory. So also remember to remove any gear from other party members you may want.

Is their a list of items that wont actually transfer over? Will the Helm of Honneleth make it?

everything except the non RTO download content will transfer over if in your inventory.  There is a chest in your castle that serves the purposes of the chest in wardens keep.
