Trying not to spoil...
I chose to not make the sacrifice at the end of the game, and I did get the other achievement instead. But when I loaded the epilogue save, I was rewarded The Ultimate Sacrifice as well. Seems like a quite obvious bug, does this happen to everyone? It was with the new game version.
