Where you got them the first time

Carnifex - from Mordin when you meet him
Vindicator - from Garrus couch in his sniper roost
Tempest - from a dead Quarian when you recruit Tali
Avalanche, Cain and Missile Launcher - Research
Particle Rifle - Horizon, just before you meet Harbinger for the first time
Scimitar - Samara's recruitment, just before you hold the merc at gunpoint
Viper - Just before you meet the second bunch of Salarians on Thane's recruitment
Revenant, Claymore, Widow - Collector ship
Firestorm - Zaeed's mission

just to clarify some of those;

revenant - collector ship (soldier only, first playthrough only)

claymore - collector ship (soldier and vanguard only, first playthrough only)

widow - collector ship - (soldier and infiltrator only, first playthough only)

eviscerator - DLC shotgun weapon, will start with it

firestorm - DLC heavy weapon, zaeed loyalty

blackstorm - DLC heavy weapon, will start with it

incisor (that right? can't remember its name) - DLC sniper weapon, start with it
