Props Surango!
Maybe bioware should offer u a job as a bug squasher. This game has made 200M dollars, surely they can scrape together 100k or so for a good bug squasher....

As one of the people who have to constantly quicksave after every battle and before every dialogue just in case of a random crash and then load and play a different character for a while when I do encounter a crash (as they always reproduce at the same moments) I am somewhat disappointed

Just played through RtO without any single crash! It seems the DirectX end user runtimes are really making the difference here, many thanks for pointing me in that direction!

To this end, I'm wondering if some files got corrupted on Steam's servers. Since the retail versions of the game are working, this seems pretty likely. How about you D2D users? I'm not really sure what companies do this outside the states, so if a nonUS digital version is still crashing after trying them please also post what country you're from and who the provider is. If you know whether they are a subsidiary of a US company or not, even better.
I do find it odd how the game's website doesn't have a link to the runtimes in the troubleshooting/FAQ section. Never seen a game with a link to them on their website, ever. Casual computer users don't know about things like this. To be perfectly honest, I forgot to install them myself until someone mentioned them in another thread. (new computer)
So, to BW/EA: If you want to save yourselves a lot of grief and angry customers, post a link to the DirectX 9c end user runtimes manual installer (not the web installer) on your website under random crash troubleshooting or something similar. Post clear instructions on how to install it as there are none on Microsoft's website. Instructions are already on this thread- copy and paste them if it pleases you.

Using a digital version of DA:O from Gamersgate. The game was solid through my first playthrough. I loaded the patch on my second playthrough (about 2/3 complete). When I tried to load from a pre-patch quicksave it ctd immediately and every time. I then loaded from a pre-patch autosave and the game proceeded fairly stable. After quicksaving and autosaving post-patch I was not able to repeat the ctd during loading a save. I did experience two more ctd and one hangup during a "loading" screen, but that was over about another 20 hours of play. I observed on one ctd that it was during a quick succession of spell castings from my mage.
While not as stable as 1.02, the game is playable with only the occasional ctd. I am using WindowsXP with current service packs and DirectX 9c.
Hope this helps with the troubleshooting!

Hmmm... I'm no admin, but you might want to beat Ortan Thaig before being able to reach the Dead Trenches. Defeat the Corrupted Spider Queen, find Branka's journal and head to the northern exit.

Those would most likely be your end user runtimes. Out of curiosity, have you tried a clean install then patch to 1.03?
Someone was saying stopping DA from sending a lot of information (profile updates and screen shots) was helping them. I usually keep those off anyway, so I don't know if it'll work.

Lots of mods are "breaking" in 1.03. Only ones that do not appear affected are ones that do not alter core code such as lock bash. You'll just have to wait for the author to update the mod. You should disable it for the time being. You may lose some loot, but it's better than losing the whole game.
If folks that use the mod Advanced Tactics haven't noticed, it can put you in a position where you cannot advance (this is one of the possible causes of the invisible wall bug).
