Never. I set it to Insanity in the beginning of the playthrough and never went to the options menu so I knew it hadn't changed.

No, just set it on Insanity at the beginning when it gives you the option to difficulty settings.

I was far too busy wiping the blood off my forehead (for a lot of times it felt like banging my head on a wall, especially Garrus' recruitment and the disabled collector's vessel) and making sure I saved after every altercation no matter how small, because My God was I not really up to the challenge a lot of times.

First time, no. It was NG+ and it was really tough. I had to stop half way through and start a fresh character. On that character, I had to check several times due to how much easier certain parts were compared to my NG+ character.

I am on my first insanity playthrough (ME1 import - not NG+) and I checked once, on Haelstrom. I let Reegar help out (renegade) and I didn't expect him to survive. I made my way down the right flank and when we got to the Colossus I was focused on making sure Miranda and Grunt where in the right spots, and I kept shooting Geth troopers and spamming AI hacking to keep the Colossus busy while I get the Cain warmed up. Surprisingly - while I was keeping the Geth off balance, Grunt and Miranda took down the Colossus on their own. So yeah, I had to check to make sure I was still on insanity.
Either way, that fight was a great moment of teamwork.
