As that manual points out, you need to run with /3GB in order for 32-bit Windows to do anything differently for an application flagged as large address aware.
Be advised that setting /3GB and /USERVA on 32-bit systems is risky as far as system stability goes--especially with a userva of 3072! (You are depriving the kernel of its address space.) I suggest not going above 2500. 2500 will probably still fix the issue for Dragon Age, so it might be worth trying. Just keep an eye out for unusually slow file operations, failed network connections, etc.

Having the loading screen issue on my X2 and core affinity didn't help at all. I noticed an interesting phenomenon though (well, interesting to me at least):
When I run the game on both cores, both cores run at 100% except for the loading screens when they drop to about 60% each. If I set affinity on the other hand, one core runs at full while the other is idle, as expected. On loading screens, however, the second (previously unused) core is suddenly troubled as well at almost 100% - and then drops back to idle when the loading is done. I have no idea why this would be, but it seems like it could be related to the issue.

XP home 32, sp3

Can the /USERVA or whatever it is be set back to what it was originally? Just in case things to start going screwy.

I wonder if this issue is due to the increased load on CPU, which lead to overheating and reduced performance. I have a laptop fan at home, and while I placed my laptop on it properly, DAO runs just fine, although the location where the CPU is still heat up sgnificantly.
When I bring my laptop to work and fire DAO up (no laptop fan), it only takes 10 minutes before the system becomes sluggish. This happens in the same load area.

I can run MW2 and left 4 dead 2 with no problems, I have 4gb ram and a pretty feeble graphics card. This is certainly a memory leak, the end of dragon age origins for me was pathetically slow and I will await the patch till I play again or buy anything else from EA.

I'm running DA:O on an i7 860 with SMT.
All together on win7 x64 prof.
Graphics Card is a Radeon 5870.
I have the same Problem with increasing load times. Especially in Orzammar.
It occurs after several minutes there. Tower of the Magi no Problems after several hours.
Seems to be also region specific.
Another Problem are Framedrops, wich aren't really framedrops.
Framerate constantly borders the 60 (vsynced) and at some point it gets choppy.
It doesn't matter if there are many effects or many people in the area. One time I had this in a tunnel with camera looking at the wall ~.~
Saving and loading clears for some time.
On my second rig I never had problems like this. Even though it's only a Core2Duo with 2.4ghz and a GeForce 9650MGT (yeah the rig is a laptop).
Playing both with 1680x1050 and full details (8xFSAA).
Switching FSAA and such off doesn't make any difference. Even switching framebuffer effects off didn't do any trick.
Switching off vsync feels alot choppier 'cause of picture ripping when getting around 140-200fps sometimes.
Also some sequences run choppy at 60Hz with vsync. Dunno why they are running without vsync without choppiness and (and that was a suprising deal) with >1000 fps (no joke).
Edit: Forgot to mention my PC is running very cool (watercooled) and at only a cpu load of barely 50% with no core maxed out (even though the i7 seems to aquire the turbowise overclock when cores are barely at 50%). Energymanagement at HighPerformance
