Shepard is actually a Reaper from the past...er...no, the future? Anyway, so Shepard-Reaper betrays his brethren and overloads himself to blow up the rest of the Reaper fleet. Unfortunately, this causes a vacuum metastability event which swallows up the entire galaxy...setting the stage for Mass Effect 4: The Prequel.

Hahaha xD but Jacks(the doctors) last name spells Shephard

Shepard sleeps with Tali and decides to retire from combat and become a accountant. He then eventually becomes old and fat and dies because of anal tearing...

Earth is actually a giant mass relay firing into the Reaper command center. After fighting off hordes of self-destructing keepers, Shepard and the Normandy fire their lasers into the stations center, instantly ending the Reaper threat. Later, Shepard learns that Saren was his father.

If Shep did the paragon thing in the end and blows up the ship and makes the illusive man who = the shadow broker mad at him and miranda was on his team and resignes from cerberus the openeing is that all the time the "secret backers" were the alliance and miranda is with him to continue to "keep an eye on him". So now he is not trusted to stop the real threat when earth is wiped out. But he does have the blue prints for the collective ship he destroyed so he must on his own get one of those built while being persued by cerberous and the alliance for allowing this to happen to earth. The "flotilla" goes to war with the geth and asks shep to help them first then he must turn his attention to destroying the harbringer fleet with what is left of the flotilla and his "collector dreadnaught" in one huge space ship battle. He defeats the invasion fleet and if shep romanced Miranda she is at his side as his "wife/gf" and the closing scene is them romancing again but Miranda assassinates him for the good of the universe. Which is what her assignement was all along.

The Reapers are only mad because Shepard broke the Prothean Beacon on Eden Prime, so the Reapers sue Shepard in Galactic Court for damages to property.
