Disk replacement?

My Xbox 360 scratched my Dragon Age Origins but my warranty ran out. My disk has a crack from the center ring and it also has 2 minor scratches. Could someone help me on what I need to mail to EA. I also don't know how much it costs to replace the disk.

If your xbox screwed your disc, EA cant or will not do anything about it. But was it your fault that the xbox scratched your disc? and/or does it do that often to other games aswell? If so you go and tell microsoft about the issue. Corrupt systems will always be replaced.
