Dual Striking; Missing?

Whenever I have dual striking and momentum on, I seem to swing and it does nothing. Is it missing? Because it doesn't come up with a MISS..
And would Heroic Offense help?

The animation is just out of sync with the actual attack, so it's not really that you're missing.

It's the dual striking bug, each time you go to fight, be sure to deactivate and reactivate it, cause each time you zone the skill mess up your attack and you miss most of the time(but there is no miss, the damage just vanish, which is worse actually).
