The whole Orghren thing sucks. You are best off not giving him a single gift in Origins, because the point loss carries over to Awakening if you do. Which makes no sense cuz his approval rating resets. I am having a really hard time getting his approval up. Loving schleets!!!!

Well two of the points mentioned in the OP are not bugs, but adding my own:
- The merchant, Lillith, is bugged inside Vigil's Keep. She asks if you remember saving her, tells you that she's spoken to the treasurer, and then she leaves the area. Whenever you re-enter the area, she's back and she repeats the same lines and actions.
- Sigrun's personal quest won't activate if her approval is above 0 or if you've accepted either the Law & Order or Smuggler's questline.
- Oghren's approval from gifts is lowered by diminishing returns from Dragon Age: Origins.
- Velanna's elven trinket gift is bugged.
- High dragon isn't lootable in the end game. Some guides said that this dragon is supposed to have two of the blackblade set. Related: 2/4 of the blackblade set isn't in game?
- Approval is very wonky. Anders, Velanna and Justice commonly are at approval "love", and companion approval isn't switching from "warm" to "frendly" after completing personal quests.
- HNF Queen epilogue card. Should it not be that she disappears with Alistair like all the other Alistair romance cards?
- Dwarf Bomb maker (may be unique to my game) accepted 2-3 Lyrium sands and then bugged out. He was unable to accept more, would not allow me to see his wares, and repeated the same line until the end of the game.

One of the axes of the original game switches a shield you wear into a quiver after every cutscene. To fix it you have to unequip and equip it again.

To the OP:
1) after killing the wraith ogre, a cutscene should begin where the dwarf activates the mechanism. Getting back to the surface finishes this quest.
2) the wraith itself isn't dead yet, there is a continuation in the room with 4 keys, the wraith possesses an avvar warlord
3) too late, it's stupid but the game forces you to go to blackmarsh last if you want both sigrun and velanna to join. Kristoff (Justice) obviously doesn't need to perform the Joining.
4) it's bugged but can be avoided by reloading and/or equipping something else just before being put to sleep. You'll know if it's bugged by being naked/dressed in clothing after you awake. If you're naked,it's bugged and there won't be a 4th experimental subject with your equipment. I got that and also had some equipment left in my inventory. Solved it by reloading and equipping something else. If you're in clothing like the others, no problem, then you're stripped properly and shouldn't have anything in your inventory.
Besides that, the bugs I've encountered were:
-ogre killing slow-motion animation bugged, the ogre falls to one side and bleeds, but my dual-wield rogue is on the other side of the screen facing in the opposite direction while performing the killing blow
-stealing bugged, it says success but doesn't give anything
-Sigrun's personal quest missed (I got her last) and approval +100 jump for just one gift or conversation, remains neutral with 100 approval and has no loyalty skills
-Oghren's approval starts at +6 per gift
-never completed the Blackblade set, got helm and cuirass at silverite mines
-after killing the Withered, my main char had his hands stuck in a very unusual position during the meeting with Alistair and the Joining until I switched weapons
-approval bugged in general, warm at +100, or love at +100 even when having a conversation about friendship with Nathaniel and Anders
-the model of the Vigilance is like Maric's sword, the Tome of Knowledge shows another model.
-didn't even talk to the smuggler so I didn't have any problems with the smuggled goods quest, knew about the problems with that one before I got the game
-told the Captain to kill the messenger after we defended Amaranthine but the credits said the messenger lived
-chose to protect everything but the Captain always said in his reports only the city is defended. Got the correct ending in the epilogue,though.

A couple of bugs I had and are not listed here:
1) There is an object from Origins "Corruption helm" which during the export of the character was changed into a rune.. Yes.. I found it as a rune crafted into my sword
2) The game not always recognized my character was a femal. On first playtrough Alistair called me "the hero of Ferelden". I believed it was just a mistake from dev, but on second playtrough he called me "the heroine".. this means the first time it was a bug

1) I've never done the orphans-quests but still got a mention for generous donation in the epilogue. Update: This is weird, I was just reading all the codex entries and the one for Seneschal Varel said he was killed during the second attack. However there was no mention of him during the epilogue?
2) Squashed the rebellion in some farmhouse, so no seneschal present, and got the dialogue option while defending the Vigil "You took an arrow for me once".
3) The Unending Flurry ability is described as being a sustained ability but it only works as an activated one. Try not to use this however, the effects of using this with your rogue is comparable to Mana Clash with your mage: one hit kills.
4) Going back to the keep after meeting Sigrun I had her do the joining. While finishing Kal'Hirol I still had the whole dialogue with her about becoming a Grey Warden.
Kinda 5) Supposedly you get help during the defense from the king of Orzammar's troops. I've never seen them.

I did not find any serious bugs myself (playing on PC, downloaded from EA Store).
One small one, a couple of the armored ogres kept pounding as if they had a victim in their hands even after they were dead and looted. It was really rather... creepy [/alistair]. So maybe it was meant to be a death twitch and was intended to be left in, I dunno.
Did anyone notice crazy things going on with their fatigue penalty in both Origins and Awakening? I was playing in Origins last night and at one point my arcane warrior had a -66% fatigue penalty, when normally she would have a -4 or 5 (wearing an item set bonus armor with offsetting stamina buff). It didn't seem to affect gameplay, it just made me go WTF. Also it seemed like my followers were experiencing higher fatigue cost of sustainables.

After you have killed the armored ogre and other boss guy at keep, when you go inside to throne room and come back outside you can loot them again at PC version (did it twice so dunno if it works moretimes). At spectral dragon fight if I use granade at start of fight the whole game crashes always. I cant talk to Ogrhen about his family even I got quest. At final fight description says that Architecs fire spell hits only on one target, even it damages all enemies. Also some random crashes when used stonefist spell. After battle at keep you cant complete Wane's special gear quests even you got mats and that bomb dwarf dont take your lyrium sand even he got arrow over his head, that is pretty mach all what pop ups in my mind now. Good thing what they did with game is that they gaved cool weapons and armors and had some cool puzzles.

These were the two bugs I encountered on the PC.
No idea if this is a bug or not : In the tactics menu there is no option to activate / deactivate Unending Flurry (for rogues), as in it does not appear in either the ability tab or activate mode tab.
Also, I was unable to ever get Sigrun's personal quest on my first playthrough, I had no idea there even was one.
The 3rd mage on the battlements bugged for me at first as well, but after exiting the city and zoning back in, I was able to target him / kill him.

When talking to Armaas (Qunari merchant) in the Silverite mines I had no option to invite him to the keep. He turned up anyway, but disappeared shortly afterwards - making 'the trade must flow' impossible to complete.
