As far as the Silverite Mine - that's a known bug, only fix is to load an earlier save (wish I had known that prior to going into the Mine and getting 3/4 of the way through) OR...you can strip your PC nekkid and save prior to entering the mine., or just put some crap armor and weapons on them when you first enter. You keep your inventory items. Think they said it has to do with the NPCs not being able to wear the armor/weaps you have (if you have some of the elite stuff)

When I put Sigrun through her Joining quest, the seneschal hands her the goblet and she speaks Velanna's audio file instead of whatever hers is supposed to be ("So let it be"). It's happened every time I do it.
When I finished doing Sigrun's personal quest, I went and accepted the quest "Law and Order" from the Captain. I ran into town in the direction of the "Suspicious Character" or whatever you call him. Suddenly, I was teleported to the roof of the guardhouse where I was automatically launched into a conversation with a guard asking me what I was doing there. Hilarious, but obviously a bug.
Will post more if I come across them.

1. "text field" and "undefined" are NOT attractive on the character screen.
2. Although I am NOT wearing quest armour - (just heavy plate) - I am still in my underwear when the game starts.
Bioware; did you do ANY testing of this before you released it...?

I don't know for the save from before entering the Fade but on my second playground I have the same bug and reloading a save from before leaving the Fade (so before recruiting Justice) does not solve it.
In my first playground Sigrun and Velanna personal quests have never triggered (I discovered that they have personal quests on the forum) and I suffered also from the Silverit Mine lost equipment bug (but I was able to solve it by striping my character before the cinematic).

I'm having the exact same issue. As soon as "Mother" starts her fiurst wave of "Special" attacks the game crashes every time.

DAO has been rock solid stable for me since day 1. Since I installed 1.03 and Awakening I have at least 2 or 3 crashes every time I play...
I have not changed any drivers or hardware; it's the game

- Inaccessible place under Vigil's Keep after spirit is released therefore cannot be finished. - Wraith's Vengeance
- Can't seem to get a lot of Wade's ingredients for his special equips.
- A lot of other stuff.
2.) Empty codexes.
3.) Extremely laggy combat when defending Vigil's Keep and corpses are UNLOOTABLE.(in this part only)
4.) Messed up questline of Law and Order especially when mixing in with both sides at different times.
5.) Instant +100 approval from Sigrun NOT when giving gift but after I talked to her in the Throne Room in front of the bookshelf.
6.) Instant +100 approval from Justice - forgot why.
7.) Even with instant +100 approval from both, the description is still the "Neutral" not "Warm" or anything better.

1) Sigrun's personal quest: the Dwarf you're supposed to bump into in Amaranthine doesn't show up if you've picked up Law and Order quest or have finished it.
2) High Dragon in Dragonbone Wastes can't be looted after it is killed.

- (Forum hearsay) : Impossible to trigger random encounter Velhalla quest.
- (Forum hearsay) : People go crazy trying to find Signur's quest as it won't trigger if you already did the Law and Order quest (which almost everyone does as their first quest out of Vigil).
- (Forum hearsay) People losing stuff in the mines when you are
stripped (probably linked to NPC experiment chars not being able to equip said items).
- (Forum hearsay) Everyone uses the Warden Armor which does not import (yes I know, not a bug according to you guys). At the very least you could start them out wearing something instead of naked
MINOR (Doh !
- Game started to lag badly during the battle with Mother but did not crash. Only time I had lag. Before that it did once crash during the battle with Mother (without lagging).
- There are multiple solutions to 'fire circle' puzzle. A hint that you're on the wrong track would be nice (for example, the circle resetting itself if you complete a 'wrong' solution).
- Lost of people get confused looking for the wraith after you kill the possessed Ogre in the Vigil cellar. A hint that it's in the previous area and will reappear when you open the crypt door would be a good idea.
- At the end of the battle for the city, I killed the Armored Ogre last. It fell to the ground with a roar and bounced. The roar and bounce cycled infinitely, making it impossible to hear the people talking in the following cutscenes.

Also I have been crashing a bunch after 1.03 both in DA:O and DA:A. The only unofficial mod I have is the unofficial 1.03 steal fix, and it crashed a lot even before I installed that.

This is not a bug. Darkspawn suddenly approach Amaranthine, therefore, Varel had more urge matter to attend to and didn't have the time to do the ritual. If you did the Black Marsh quests last, you should be able to do the ritual for both Sigrun and Velhalla.Louis deGuerre wrote...

I believe it happened the moment they joined up (Justice and Sigren) or maybe when I gave them an appropriate gift. I am not sure anymore. It might be that they all got it the moment they joined up and I only noticed this when I had given them a gift. Many people have reported this btw.

I agree, but it is bad game design. It would not be hard to code something like :
if (still outstanding companion quests or events) then do not trigger endgame.
and save players a lot of frustration.

2) Bugged approval rising for Sigrun, Justice and Nathaniel - ive gave them one item and theyre approval rating was raised by +100 but they remained on status neutral
3) at Wending Woods map, while exploring the part of map before going through the bridge you will sometimes get teleported to a guy laying injured on the other side of the map. This breaks the scripts for Velanna as she will stand on the cliff doing nothing
4) Skills obtained from Wardens Keep dont work, in skill tab theres a message stated: {class name not found} (these were two unique skills for warrior)
5) Cant do the joining ritual on last party member, as there is only option going to war.
