This issue happens on an Italian version of the game, so i don't know if it's common to all of the other versions:

There are two quick time events happening in the game so fast you can barely see the simbol appear.
The presence of the event is made clear by the sound, but it happens in a blink and you cannot press it in time.

I can't remember where both of them are (i hope to find it again on a following playthrough), but one is on tali's loyality mission: when Tali is crying on her father's body a qte happens so fast you can't take it, right before the end of her dialogue.

I even loaded the game twice to try and take it but it's simply impossible, even by keeping the paragon button pressed or by spamming it through the whole conversation.

I've noticed the one during the Tali's Loyalty Mission. I spam the button during a second playthrough and i have activated the sequence. Shepard gives a hug to Tali. I beleive the more paragon you are the longer it's available...
