As I mentioned I did not save the Vigil, I didn't even fully upgrade it but all my companions survived while I saved the city. Seeing so many other people say their companions died makes me wonder what I did to trigger all my companions survival.

From what I've read, there seems to be a "happy" ending. The companions survives while defending the keep and you save the city. I think that the companion reputation plays in a lot here, affecting their performance- the higher the better. The ones I left behind were so-so and therefore, ended rather poorly.
Edit: Corty, did you send soldiers to the farms? Or to guard miners? Did you have high companion standing with the ones you left?

I upgraded it to the max, but sent off soldiers to guard the farms and ****, so I felt it was my responsibility to save the city! I really thought that by having the keep fully upgraded, I would be able to do both. It seems not. Justice got his head chopped off, the keeper was buried (but disappeared), Sigrun led a suicidal but brace charge. Vigil was overrun but not before slaying a billion trillion darkspawn.
I wish I knew all the deciding factors for this. Does the party setup you leave matter? Perhaps their standing with you matters? Seeing as how I had barely bothered to bring or raise any of the ones I left behind (god they were boring), that might've been the case.
