1. Sound cuts off during some cut scenes or it stalls. This was very obvious in the opening cut scene with Oghren. I thought my game had frozen.
2. Game froze twice. Both times during heavy combat.
3. Blank codex entries. I thought that they were just separating the entries, but then I noticed that some were indeed missing.
4. Random encounters don't seem to trigger very often. I only got one merchant and some Drakes. The Drake encounter was horrible. My map in the right hand side was just a a black circle with a static white cone which should have been the direction I was pointing.
5. Vigilance and Sentinel did not display properly. They showed up like DLC content.
6. My rogue did not detect any traps even though she was maxed out and could detect traps like crazy in Origins.
7. It was really annoying not being able to complete one of the joining rituals. If that dialog option was going to be removed once the end game started, they should have made the Blackmarsh the last forced stop before the final battle.
8. Certain loot does not drop no matter how many times I kill them or what level. This is obvious with the Apostate Mage and especially the High Dragon.
9. The last bug that I can recall is in Epilogue. I did not even romance Alistair in my Origins playthrough yet it says he's my "true love". I should have gotten the Liliana epilogue as some people have managed to get it. They might be on PC though.
These are the major bugs that I can recall. It was enjoyable, although my character did feel like a god just walking into these fights and decimating the battle field. I think the game could have been made a bit more difficult and had these bugs been fixed it would have been more enjoyable. Part of me felt like I was paying Bioware to Beta Test.

Vigil's Keep Interior: sound effects missing (swords, footsteps)
Random encounter with drakes in canyon: map is blank. If you open the inventory or character screen in that encounter, the game crashes
After one of the Vigil's Keep cutscenes, my character's arms were outretched and would remain so during normal play. Earlier reload fixed it.
I romanced Alistair but had him become king and marry Anora. Stayed as his mistress. Yet I got the same epilogue card as if Alistair hadn't become king.

The heroic accomlishments screen has no monster listed as the strongest creature slain. It also says that I have explored 100% of the land, even though I didn't...
During the Darkspawn attack on Amaranthine, the merchant in the Chantry has absolutely nothing for sale... at all.
Also, although I killed every single enemy in the first two sections of the area under the keep. The quest (which is in the Active Quest area) says that the path is open and that there are Darkspawn to kill, but I can't get past the "Blocked path", and I can't find anything else to kill...

1. Within Vigil's Keep at the beginning of the game the sound effects would turn on and off, sometimes it'd lag and play in the background of my cutscenes, for example, when I met Anders all you could hear above his dialogue was sounds of metal clashing and darkspawn being owned. When meeting up with Oghren, most of his dialogue isn't heard.
2. Blank codex entries, as a few of us have mentioned there is no way to list them as they have no title or text entry.
3. I made Anora queen and yet it's Alistair who comes to the Keep, he also claims to be my husband, which throws a whole lot of confusion into the mix considering I;
a) Didn't romance him in this playthrough
4. I've no idea how many random encounters there are, but through my playthroughs on my warrior, mage and rogue, I've only ever just done the encounter to save Lillith.
5. There's a whole thread dedicated to this, but Sentinel and Vigilance skins aren't displaying as they should.
6. I've maxed my lockpicking skills and yet for the life of me I can't unlock certain chests yet Nathaniel and Sigrun could.
7. I have an issue with my epilogue on my mage, where it claims I returned to court to a grinning Alistair, where in fact I should of triggered the ending with Zevran, if such an ending exists.
I'm playing on the EU download version.

1. I only noticed this mostly with Justice, but when he had some of his sustained talents on (a shield one and a spirit warrior one), he becomes transparent-ish. I can see like his teeth and eye balls and a translucent frame of his body, but I can see right through him and his facial features are gone. This might have had something to do with him being also being equipped with the Sentinel armor, which is apparently bugged as well (looks like Warden Commander).
2. Vigilance looks like starfang [I do have Warden's Keep, but haven't tried disabling it].
3. After I defeated the Mother the first time and the end stories played through, the scene right after Mother is defeated replayed again (where you run and jump and stab her in the face), only slightly more glitched out. It played through twice more and each time it lost more and more continuity -- the characters were moving around like chess pieces instead of walking, and some other weird stuff. I think it would have kept playing indefinitely, but I started mashing buttons until I got sent to the start screen.
4. I also noticed that my rogue almost never detected traps -- and when I did I think it was actually Nathaniel. I had all four points into the device mastery tree.
5. I was able to pick the silverite mine door prior to getting Velana, but fortunately I didn't go in.
6. When I first got to Amaranthine I was approached by the shady character and pretended to be interested. After I talked to the guards I decided to flush the smuggler's out. However, after they were all dead, the quest flag for the Crown and Lion stayed up and I still had the option to tell the innkeeper to open the passage. I tried telling him to open it just to get the flag to go away, but then everyone hated me, so I reloaded it.
7. I'm not sure if this is a bug or not but that game seemed to have a noticably harder time handling large battles -- it would slow down and take longer to load [than Origins].
I think that was it for the most part -- maybe some other minor graphic issues, but I can't remember the details.

The Heroic Accomplishment section also doesn't list the most powerful creature slain and it also shows 100% world explored.

1. Bowl lanuage somtimes the dialog get faint and it litterly sounds like there talking through a bowl
2. Texture loading lag when you play for like..10hours (Example:) Inside vigil aka camp its a completely black room.
3. plus you gotta reset quit game and reload if you dont the load times get longer and i even had moments where places didnt fully load.
4. DLC from wardens keep and such doesnt work not a bug and not a big deal with me but a issue we pay money for this stuff!
5.My map goes completely black at times if i play the game for long peroids
6. Some minor framerate problems although the inferno spell seems to run a bit smoother on awakening.
7. Alot of big spells still lag up your game massively 2infernos 2 tempests and 2 blizzards on the last brood mother lol...
8. oh and when i gave velanna the blank journal i returned to the vigil shes telling me whats she gona do with it but im not looking at her instead im looking at the middle of the room just like a screen shot but i was choosing dialog and what to say very strange so quit your game once every 4 hours or you could have massive loading lag times with everything.
9. Sound effects cut out when assaulting vigils keep right after anders just the blade slashing sounds
thats about everything i can think of bug wise awakening is a good expanson pack but had way more potental as almost a new full blown game this is the only game where i can turn the other cheek and deal with problems like this cause its just so well written and the next game will be way better and refined mark my words!

Warden's Keep DLC - runed or not runed, human Vig and Sent armor both look like WK armor/starfang
Face textures disappear in dialog for main character in the last 5 hours
Sound stops in dialog, must press square to advance.
That 2Hand skill bug, I forget the name the activated mode w/ skull icon. When the CPU activates that the PC toon get's stuck in that animation and will not move or do anything else unless deactivated it. LOL happened on the final boss, I ended up using Nathaniel for 80% of the damage using archery while my uber 2H PC toon swung at nothing for 3 minutes
Tonnes of blank codex entries, these also show up blank on my Persona character page (which is bugged, does not show all the skills/abilities)

I had this happen to me with Oghren during the final boss fight. At first I thought he was confused or something but after whatever negative status effect wore off he was still just swinging around during nothing.

Yeah basically, don't use that mode and don't set it in tactics is the workaround. Since it was the last boss, a lil late to respec oh well

Also, the High Dragon does not seem to drop any loot. I'm not 100% sure if this is a bug or not, but several people have stated that it is supposed to drop pieces of armor.
