You get the chest from the First AFTER you complete the Fade part of the Quest and are returned to the real world, also you can no loot those chests that have the other peices.
When you are in the fade there are three areas guarded by desire demons and defeating them makes a corosponding chest appear in that spot when you return to the real world. This must be done before you leave teh fade/fight the baroness because you cant go back without cheating and that option is only availible on the PC version.
As for fixing Vigilence and Sentinal theres been mod for the pc for a long time that work to do this. For teh consoles you HAVE to delete Wardens Keep and have a gamesave or character made that never used that content otherwise you will still get a error.
