Redcliffe can be a serious pain to do depending on what resources you have available to you in terms of potions, people in the party and their abilities. But add a level to your party, have a couple of spells that incapacitate multiple opponents, have some good luck in places and it is do-able. But that doesn't mean it is easy or you won't sweat a bit. I don't look forward to that battle at all.
It is probably easiest to do that if you are playing a mage and if your party has two mages total since that allows you to keep the militia healed as well as your party.
One key thing is to just pause every few seconds and re-assess what is going on. Pause is ultimately your friend.
The final battle in the deep roads I've done 3 playthroughs on hard now...in total probably 10 times given the unsuccessful passes and the 1st playthrough I dropped the difficulty to easy after 5 or so attempts that was a bit too much, then the 2nd playthrough I dropped it to normal after 2 or 3 attempts and this last one after 3 frustration filled attempts finished it at hard (rogue, Zev, Morigan and Ohgren) with Morigan dieing to a shield pummel and assault.
Worse then this for frustration is a side cavern with 4 corrupted spiders and 2 genlock emissaries. That is just absurdly annoyingly difficult.

4. The battles were tough, but I count difficulty by numbers of failed attempts! I have none!
As for the difficulties go, I personally do not play any higher than casual unless I am fighting certain boss battles, just to keep it entertaining.
None of the above.
5. I was able to defeat most battles, even boss fights without reloading even once. I never had to reload for any battle more than twice.
I think that qualifies as "no problem."
Like I said though, my first playthrough was on hardcore and I found that challenging at times (4+ reloads for a couple tough fights usually closer to 1 reload per hard fight). Once you know what to do, you know what to do. Nightmare isn't much more challenging than hardcore.

Bit of both: It's a hard game, but once you get a better grasp of the mechanics Nightmare can be a cakewalk.
Instead of Focus Firing, use stuns, disables and AoE attacks. Control Aggression effectively with your Warriors. (Try not to have more than 3 enemies attacking a single character.
Lower the difficulty down if necessary, and don't forget to use items. (Most useful ones off the top of my head would be Acid Flask, any grenades, Rock Salve (for tanks), Swift Salve (for Rogues) and Deathroot Extract. (Should try to get poisonmaking on any melee using character ASAP))

I think that's the most difficult battle throughout the entire game. All other battles pale in comparison. Next playthrough I'll kick up a Storm they won't forget in a hurry.

Maybe it also just depends on what class you are playing as, i played as a two hander my first play through and put almost everything into strength and had really no problems in the game whatsoever, especially when i got haste

The battle at the entry to the Frostbacks is a test of "Are you high enough level to proceed."
yep, I went there at level 7, failed, tried again at levels 8,9,10, 11, got killed, tried at lvl 12, killed all enemies all right, no damage to my party whatsoever

the final battle in orzammar is a joke, it's the battle BEFORE the final battle that is the most frustrating and most difficult battle I've seen so far (the battle agains the spirit apparatus). In order to do this properly, you need to dispatch 3 waves of lieutenant rank enemies consisting of 4 enemies each , killing all enemies SIMULTANEOUSLY, otherwise the remaining enemies will just respawn at full health. And then there's also the spells the stupid apparatus casts (heal and drain health). I used 3 entropic deaths and the nightmare combo amplified by affl. hex and vulnerab. hex on each wave to kill all four at once, then quickly disable the altars. This way only 12 enemies overall will spawn. After each wave I disappeared into the force field for several rounds to regenerate spells and mana.
3 mages required for this battle to pull this off. I am proud to say that I only lost 55 points health and Leli lost 19, and only due to the spirit apparatus drain health, but since I can't fight the apparatus directly, there's not much you can do about the damage, and it's also irresistable damage (anti-magic ward doesn't work), so completely and utterly unfair, Bioware is so frustrating!.

Anti-magic ward on Leli and the warden, Leli goes stealth, gets close to spiders, casts captivating song, warden runs in, casts sleep on everybody, casts mana clash on emissaries, casts affliction hex and blood wound on spiders, casts death cloud (get Leli the hell out of AoE), all enemies are dead

Trying to save the beards on every Dwarf in your way to glory? Or Inferno & Fireballs? Tired of long treks and Dwarven politics? Or Blizzard and Paralysis? Trying to save the lives of NPC henchies that were formerly trying to kill you? Or Storm of the Century?
Maybe Evil does have a place in the game after all....
