Maybe it has something to do with the destructible container they're hiding behind still being an obstacle after it gets destroyed?

On the flip side of this bug, has anyone used Charge on a bull-rushing Krogan and sometimes not have their cooldown trip? I've had it happen a number of times and its always a pleasant surprise. Take no damage, stop their rush and can use it again. Its happened multiple times in the fight against Garm and Uvenk.
Charge into rushing Krogan (who still has some kind of protection)-> staggers both parties -> cooldown not tripped -> hit it again (refreshes shields)
Has anyone else gotten this?
I get that exact bug in my Thane Recruitment video. I am able to charge the end Krogan twice when he is charging me.

Though the most irritating though infrequent bug I have is when I knock a target into the scenery. Not bad in most cases but when you have to kill all the targets for the door to open or for Samara to advance with the biotic field, totally irritating. Happened to me twice with samara being the shield person at the point with the husk swarms and scion. Both times on my adept and that is maybe the toughest fight for me with my adept, until I got wise and used my sniper rifle and hip fired through most of its ammo.

Pretty much the same thing often happens when you are fighting Scions from close range and charge just as they start their close-range attack. Charge works, but does not consume cooldown, so Shep can charge again.

Yep. I can't seem to replicate it at will unfortunately. It has saved my butt on a few occasions where having to eat a krogan bull-rush would have probably lead to a death.
I wonder what the underlying mechanical reason for that is. It may be useful to figure out why it happens.

I noticed that it will bug out across map transition boundaries as well, 100% of the time. As an example, in Zaeed's loyalty mission on Zorya, the first time you see Vido on the catwalk and Zaeed sets the place on fire, you need to kill two mercs after the door opens. You cannot charge through the doorway/map transition.
Other times, you're meleeing something, step back, the opponent is staggered and you want to recharge your shields. The opponent is 1.5m in front of you. "Can't get a lock!". You frigging blind! ...death scene....
On an unrelated note, when playing the Sentinel and using throw, I notice that maybe 10% of the time, the throw hits, you hear the throw sound, the opponent lights up with a biotic glow, and yet doesn't move. This happens clearly on opponents with half-red health (zero chance of having shields). Just another bug. Has caused Shep's death too when you're counting on it to knock an opponent over (YMIR, Geth Prime) when you're closing in for the kill, and it does nothing and you get minced. 9 times out of 10, it works, it's that 1 time when it bugs out and usually it seems to occur when it's most critical that it doesn't fail.

Cant get a lock doesn't happen too often and I can sometimes predict where it will happen. I have never been able to charge into the alcove with the 3x LOKIs in it on Cerberus Station just before Miranda shoots Wilson. Its as if theres a gap at the threshold which the game registers as unchargeable. I dunno. The collector platforms don't bother me since its predictable so you can work around it.

Grunt recruitment, there's a couple of heavies across a gap, and I charged one ...
For some reason I ended up standing on top of their cover (with just my toes on the railing), and promptly fell down to some bits of floor with no exits.

Yeah. Almost all of the videos I see for this section have Vanguards charging from platform to platform. I can't do that. Shepard shoots forward over cover, then stops before leaving the current platform. And dies. I'm on a 360 though, so that could be part of it.
