Two: Where did Starfang and my downloadble content go?
Three: Where did my shoes go?
Four: I died at the end of DA:O, am I just, alive again?

Two & Three: All DLC, minus RtO, isn't imported to Awakening. If your shoes were a DLC item, unless they were Cailan's Greaves, they didn't get imported either.
Four: Yes. Bioware is 'hand-waiving' the US.
