I can't give this warning without minor spoilers. So be warned.

When you enter Silverlite Mine all your gear is taken away from you as a plot mechanic. You get your gear back by escaping prison and fighting 3 enemies to get 3 sets of gear back. The last set of gear (main player char) and the misc inventory items are supposed to reside in a chest right outside of the room where the final boss is.

If you imported your character from Origins there is a large possibility that the chest will be empty and you loose the 4th set and misc inventory items. Apparently it happens because of some DLC items from the Origins campaign.  Which ones is still unknown. But for the reference I had Wade's superior Medium armor and Helm Of Honoleth, not sure about the weapons and jewelry, but they get lost too.

So be sure to save your game just before you transition to SIlverlite Mine! The game will not do it for you!

My only solution was to take clothes off of every char and put it in the storage chest, along with all the items and go completely bare-assed to the Silverlite Mine. The fight is much more harder with the few items that you collect before the boss fight (even on easy) but at least you don't loose any items.

Agreed, I had to fight four ghouls to get everyone's equipment back... maybe you missed looting one.

Yeah check all the dead bodies. Or look around in case you missed an enemy.

hmm interesting that the Helm of Honnleath is on your character, since it that was Stone Prisoner DLC gear. So running a Mod I am guessing, and that might be the issue.

No, the Helm of Honnleath is just carried over from DA:O
I had it too and I dont use modifications

(and it already has been confirmed that some DLC stuff actually is imported - some is not)

oh and btw:

I got the same issu in the mines - my mainchar lost his gear, but I didnt mind in the end when I got  new and much much better ones

As posted loot all 4 bodies. Loot chest near door and X quest variable. Also loot everything possible for the $$$.

In my first playthrough, my main character still had his gear after being tossed in prison, except his helmet - which vanished forever. Only found 3 ghouls and no chest. From the variations I've seen reported, I'm definitely thinking this is a bug - but one of those whose effects vary from game to game...

It's bugged for some people, probably having to do with equipped DLC items. See this ongoing thread in the spoiler forum:

Yes its bugged. Sometimes the items you have in your inventory stay there and the items you wear before the prison don't appear.

Yesterday I tried it two times with exact the same character. I didn't change the gear or other things.
The first time the bug appeared and I lost my items.
The second time my complete gear (inventory + my personel items) were in the chest at the end.

There must be some heavy bug with the script that is triggered there.

Another major glitch- yeah I got this one as well. The problem is, bioware likely won't fix them for months...There are a lot bugs though, so I guess it's no easy challenge....

I lost the entire sentinel armor set through this bug, and have no saves outside of the mines.

Helm of Honnleath is not core DA:O it drops from Kitty in the Village of Honnleath as part of the quest to recover Shale.

Therefore it may be DLC that is not coded to carry over, and is doing so, thus causing this bug.

The only DLC that is supposed to carry over is the DLC from RtO.

It might also have to do with any items you may have created or added to the game yourself (custom content).

I had to reload and re-do about an hour's worth of content, then saved right before being captured. I then went through this sequence about five times, each time ending up naked in my cell and bugged.

Then I tried stripping myself naked, so that all of my main character's gear went into the inventory. Lo and behold, this time it worked correctly and I woke up in my cell wearing clothes.

What ends up happening then is that the fourth ghoul that's supposed to be wearing your gear instead wears some default equipment and apparently cannot be looted. Your main character's stuff will be in the chest near the Qunari vendor.

SPOILER ALERT:

First time through: Hero woke up in prison bare naked except for underwear. Companions each wore a prison cloth garment. I STILL HAD all my inventory items less the gear I had been wearing. I progressed through the mine and retrieved all companion gear, but no Hero gear at all. The chest at the end was open-able, but empty. I am a rogue and lost Helm of Honneleith (which I believe may activate the bug), Felon's coat, Red Jenny Seeker gloves, Wolf Tread boots, Key to City ,Ring of Ages, Andruil's Blessing belt, Voice of Velvet dagger, Roses Thorn dagger (daggers had 3 GM runestones each). Needless to say, I was crushed. It was tough killing the dragons with lousy gear.

Extensive searches and reloads starting in prison and at other points in the mine were unsuccessful in finding my Hero's gear. On one try Justice's "experiment" character would not turn hostile, so he could not be attacked or looted. He even resisted fireball AOE spells.

Then, starting from a save outside the mine, I stripped the hero's gear into inventory, and re-entered. I awoke in prison wearing prison garb with nothing in my inventory except the quest items. Rather than plod through the mine again, taking a chance that I would lose my gear, I decided to reload and store my stuff at the Keep.

After reloading a save from before entering the mine, I went to the Keep and stripped my hero's gear into the chest. I also filled the chest with as many other inventory items as would fit. I still had a number of items in my inventory. I went back and reentered the mine. This time my hero awoke in prison cloth garb similar to my companions, but my inventory was empty for except quest items. I killed the three "experiments" and got all my companions gear back. I also killed a fourth "experiment" but was unable to highlight the corpse to loot it. My inventory items WERE in the chest near to vendor, and, of course, my personal gear was still in the storage chest at the Keep. Breathing a sigh of relief, I continued the game.

I have a hunch that storing only the Helm of Honneleith at the keep might have avoided the bug problem. I may try that when I have the time.

Know that if you awake in prison naked and with your inventory items, you will probably experience the bug. I would like to hear other players experiences.

Items from both RTO and Stone Prisioner carry over. My character is wearing the Helm of Honnleath. the Key to the City, Lifegiver. Olaf's cheese knife, Dead Thang shanker along with Wade's Superior Drakescale armor. I experienced no bugs in the mine. The fourth experimental subject was wearing my warden's stuff. Oghren was using Calin's shield and Duncan's sword. The other experimental subjects were wearing my companions stuff.

The rest of the stuff was in the chest.

I did this and got all my gear back. The 4th experiment was with the 3rd.

Off the top of my head I had the Helm of Honneth, Cadash Stompers, Felon Jacket, Voice of Velvet, The Rose Thorn, and Jenny Seekers on my PC rogue.
