First of all, do all the loyalty missions. A loyal squadmate has a much better chance of survival than an unloyal one.
The tech specialist in the vents must be Tali or Legion. No one else is able to do that part right.
Your Fire Team leader must be either Garrus, Miranda, or Jacob. You don't have to stay with the same choice during the whole mission, but the squad leader must be one of these three. They need to be loyal because if your fire team leader isn't loyal than your tech specialist could die on the first part of the mission, and the squad leader could die on the second.
The Biotic specialist must be either Jack or Samara. They're the only ones with the biotic power to carry the barrier through all the way. Remember Loyalty is your friend.
For the escort you can choose anyone but if the escort isn't loyal, then said escort will die along the way. Most times the remaining crew will die as well. I recommend Mordin for this job if your not taking him to the final boss.
Now for the tricky part of Hold the Line:
The people you don't take to the final boss will be hunkered down and covering the door, so you need strong defenders. Everyone has a HtL score, if the squadmate is loyal the score is higher.
The defenders with the highest scores are:
Grunt
Zaeed
Garrus
The defenders with the lowest scores are:
Mordin
Tali
Jack
The game will add up the total scores of you defenders and check them against a number it decides (please note that easier difficulties have easier checks) if you pass the check everyone lives, if you don't the game decides who dies based on loyalty, and HtL scores.
The squadmates you take with you are not counted as part of the check.
The squadmates you take with you to the final boss can be anyone, but if they are not loyal they will die. If you are going for the Nobody Left Behind achievement, I suggest to take people with lower HtL scores.

Who do you recommend?
I lost Mordin during the Hold the Line part the first time I played and I was pissed off, so I've been sending him back with the crew every time.
I also lost Jacob because I took him in my squad through the Seeker Swarms and Miranda failed to hold her barrier all the way. Started using Samara ever since, and have had no casualties since.

The only reason I didn't recommend a squishy to escort the crew was to let him decide on who he takes with him, as it doesn't make a difference as long as the rest has been done right.

along - not sure if it has definite impact)
Biotic: Jack, Samara/Morinth
Escort: Anyone (best to send a squishy, like Tali or Mordin)
Defence: Grunt, Garrus, Zaeed
Final Party: Anyone
ANYONE YOU CHOOSE IN ANY OF THESE ROLES MUST BE LOYAL.
If two members had an argument (tali/legion and miranda/jack), and you couldn't solve it via para/ren then you lost someones loyalty.

Everyone should be loyal. If they're not, then there's a good chance that they die durig HTL unless they're in your squad. Also, Grunt has the highest HTL stat so sending him to escort the crew weakened you for the last part of the mission.

I think I get it now. I don't think I picked the right option, because I only had to select who went with me on the final mission (Garrus and Mordin). So, if I pick the other choice (leave someone to defend or words to the effect), I pick three who will be in that group?
I picked Grunt for escort because I wanted to make sure the crew made it back, but I take it that anyone loyal will survive that mission.

Next time, use Mordin as the escort. Also, when selecting for the final party, take people who aren't combat savvy with you (like Tali). I took Miranda and Tali on the final run and everybody survived.SuperMedbh wrote...

Their "combat strengths" will be totaled up, and if it's insufficient, someone will die.
Priority for this death goes towards the non-loyal, but it's also the reason people lose mordin ('cos he's high on the squishy list).
So, taking the scenario with a non-loyal zaeed as an example, the most likely way to have 0 deaths:
Send a loyal squishy (such as tali) back as escort.
Take 2 loyal squishies (such as mordin and miranda - she's nigh invincible up to this point, but it seems her combat score is low) with you to the final battle.
Also, it seems grunt has the highest combat score.

Ok this is my fifth time trying the suicide mission. All of my crew is loyal and I tried Miranda as second squad leader and I've tried Tali and Legoin as techs but they keep dying. I almost have every upgrade possible. If it doesn't work I need to contact bioware because there is something seriously wrong
When Tali and Legion had a "chat" after Legion's Loyalty mission, did you choose the renegate/paragon sentence to calm them down?
If not, the problem is there.

Final Battle team: Zaeed, Legion.
I play 360, I haven't lost a team member since my third run, because I didn't get Tali, so Thane died because of unupgraded shields.
