On my first playthrough with Awakening I used my 24th level Dalish elf rogue which made the expansion easy.
I am now on my second playthrough using my first character a 18th level human noble rogue. I was playing with this warden on easy to get use to the game mechanics in DA:O and before any of the DLC. It is a different story. The combat is tougher. If you play as a human noble then some of the dialogue options you get with Nathaniel are different also.

Just finished Awakening on Nightmare. It was a great expansion.... totally worth my time and money.
The quest areas / environments (e.g. Kal Hirol, etc.) were equally good or even better than the original game. Also, since you start at a high level, you get to experience your favorite high end spells and talents from the start. In DA:O, you only got a glimpse of it towards the end. Also, if you ignore the abrupt ending part, the last two phases of Awakening (battle and what comes after it) was much better than post Landsmeet part (battle of denerim) of DA:O.
The bugs were there, but nothing game breaking. The companions were more like assistants (not friends), a few of them with great sense of humor. I don't necessarily see it as a bad thing... just another way to do it. Except, they could make Oghren belch less often.
I think they could have made the nightmare mode tougher than its current state. The final boss was all right, but rest of the game could have been made tougher. The first time I used any healing or lyrium potion was in the final boss fight (playing with Blood Mage, Anders Healer Mage, Sigrun as archer and Oghren as Tank ... not using any of primal aoe spells or death cloud).
