I only seem to be able to use the middle mouse (with or without Alt) to pan and rotate in the level editor. It doesn't matter which camera I use (Windows or 3dsmax).
According to the wiki I should be able to use the RMB. I also follwed the suggestion in the wiki and looked at the DefaultSettings.xml file but it is quite confusing as to which secition does what. What is the difference between the <Windows> section and the <WindowsCamera> section? They seem to have similar settings. I have tried modifying the camera rotation properties in both of those and nothing changes when in the windows camera mode (after restarting of course).
Is anyone else having this problem?

there is actually three camra modes, windows,3dsmax and WASD, you can change them all to the same unless you want to beable to toggle. that way you can be sure it behave as you want.

Well I seemed to have managed to edit the 3dsmax one. There are actually only 2 camera modes in the level editor (not area editor). Maybe I was confused about which one was the WASD camera in the settings file. Also you have to be careful not to set the camera movement controls to be the same as a selection command or it won't work.

Use ALT+hold down middle mouse button to rotate the camera.

the middle button is usually the crappy scroll wheel on most cheap mouses so just replacing most MMB with RMB will work. that and better keyboard camera controls was needed before i could even work with the level editor. so editing DefaultSettings.xml is a must imo.
