Sure he did. It said, import which character and he picked "Dead Guy". Bioware gave options and he chose poorly.

The game is completed either way; being a dead warden is not a bad or poor choice.
Pick a dead warden file and the decisions made by the dead warden are the ones used for the Awakenings story; either replace the Warden with an Orlesian or bring the Warden back to life and mention that the Warden was brought back. Bioware paritally picked the latter but for some reason did not elaborate on why there is a dead Warden walking around.
Better yet don't even allow a dead Warden to be imported.
If the player can ignore the fact their Warden is dead, so can Bioware is a cop-out. Any Warden, every Warden you can make in Origins even those that have not completed the game can be imported into this expansion. Bad coding that lacks proper data integrity checks and restrictions.
