My theory is if you take Thane and Solus with you to fight the reaper, and everyone else dies, everyone will be dead by ME3.

I doubt it... but I never finished my FAIL playthrough yet. (but I am working on it!!) It's a shame he looks so awesome...

I think that if everyone, or close to everyone (I've seen games where 1 squad member makes it out) dies then Shep dies. No stopping it.
