well, I don't know whether it depends on how many point I can get from paragon or renegade. but I always have loyalty gain or loss problem between Miranda and Jack (or Tali and Legion fight after loyalty mission). I don't have a neutral choice to keep their all loyalty. one of them is just lost anyway. I also disappoint with ME2 character AI. I brought Miranda loyalty mission with Jack. after finishing it, they fought in Miranda's room anyway. it's just stupid. I wish that AI would be more smarter than that. I guess that there is no option to keep their all excepting increasing a lot of paragon and renegade. now, with new character on Shepard (8th game) as infiltrator, I am already in 30 level before going to suicidal mission.
I suggest that Bioware should develop smarter AI for not only actions but also for conversations. it's just pain my butt.
the relationship is always bad.

It's not an AI problem, that's part of the game. Once both Miranda and Jack are loyal, they have a conflict in Miranda's office. In order to keep the loyalty of both of them, your paragon/renegade score needs to be very high (90% at least).
Even if you lose the loyalty of one, you can gain it back by talking to them. But if you lose the loyalty of the person you want to romance, only loyalty can be regained - the romance is lost.

I find that the Miranda/Jack conflict has a lower Renegade requirement, while the Tali/Legion one has lower Paragon requirement. At least, it always seems those are my only options in my fairly balanced Paragon/Renegade playthroughs.

No, because I kept their loyalty with far less paragon than that. I had about 60% paragon and 30% renegade, so it depends on which point of the game the confrontation happens. If you do both loyalty missions early in the game, you'll need a lower score than if you do it later in the game.

I've never had a problem with either. I get all recruitments done (minus legion) before I start on loyalty missions and when I get to the point of going after the IFF that's when I start hitting loyalty missions, as well as finishing up side quest getting my upgrades done etc. Played through at least a half dozen times this way and I keep loyalty of all 4 of them and can romance whoever.
