So where is the 'master lyrium potion'? It's required for the golem's might quest, as i understand, but i don't see the thing anywhere and I've had two playthroughs so far. Is it a recipe? Where do i find it?

The recipe for the master lyrium potion is found underneath Vigil's Keep where the Howe crypt is with the 4 keys you must unlock.

i lost the recipe in the silverite mine (don't ask how). is there another location?

If you have the console enabled you can get it from the fab room by typing "zz_fab" at the console prompt.

You can't lose a recipe... It's automatically 'installed' so you can make it. There is no other way to get the potion, except possibly importing from Origins or cheating with the console.
