Does saving or kicking him into the lava have any bearing on the rest of the game or epilogue? Also what does he actually give you as a reward for promising to free him, never know which new item it is in my inventory?

I think not sure, but I think Stefan may intensify the disease effect in the city because when you free him he is at Amaranthine. You can ask him if he feels ok and he just gets scared and runs off. Just speculating, my first run I saved him, second run I killed him. Will look for any changes.

If you opt to let him out without asking for a reward upfront, the game rewards you with a sovereign.
