Okay, I clocked nearly 50 hours on my main game save, beaten it 3 times, though I haven't completed all side quests I've completed most. I'm having trouble discovering 100% of the world. By the time I reach the arch demon it says I have I believe 93% completed. I have the guide book and looking on the map I have all possible locations for the exception of The Brecilian Ruins, (those who have the guide book refer to page 154-155 indicated as location #23) I do believe that this is a mistake because I cannot find it after hours of search... now there is a Brecilian forest and an Elven Ruins but no Brecilian Ruins though the guide book says otherwise. Could someone who has the achievement please help me understand what I am missing. Before I hit the Landsmeet meeting itself I have 78% complete... Can anyone think of anything I may be missing?

You wouldn't have beaten the 'Nature of the Beast' questline without going in the Brecilian ruins, I believe.
I have everything covered that is in the 'guide' map thing and can't get the achievement either. In fact, I've made sure to step in _every single door_ in _every single area_ so I get _every single loading screen_ and still haven't gotten it. The only thing I can think is that you have to go in all the unique areas for each origin also, but the achievement does not specify whether it is across multiple playthroughs or not. I've been wondering if it may be broken.

Looking around the internet this problem seems to be pretty rampant. A little frustrated there's been zero official word on what the deal is here, plenty of people seem to be getting it who shouldn't, while others are getting screwed for no reason whatsoever.

Commonly missed part is the extra Thaig opened up through Shales personal quest.

The DLC doesent count for the achievement, i dident do it and it have that achievement, you have to finish the game to get that achievement, well atleast to the final boss

I would like someone who made this game to let us know the requirements for the achievement. Why is it guessing game to get this figured out? THis should have been included in the game guide and is not. Please help the people who bought this game!

Ye it also happened to me and the Master Warden and Blight Queller trophies dont work I have 3258 Darkspawn kills

I've been wondering if I have to do the other character origins too, along with all the Crow assassin quests in case they lead me to a secred hideout or something. And yeah, I'm also the person who goes through every single door and explores every inch of the map. I also do all the chantry board quests I know will take me to a different area on the map.
As for Blight-Queller and the other one, now that you mention it, I have been a little suspicious as to why I haven't gotten those yet, 'cause I definitely qualify.
