They happened when I was cast into the Fade by Sloth, near the end, when you get to visit the Nightmare places where your party members are stationed (My party at the moment was: Wynne, Leliana and Alistair).
When I reached Leliana I talked with her, exposed the demon and therefore I killed it. So far so good, but when I spoke with Leliana a second time the whole conversation "restarted", taking place as if the demon was still alive (although he didn't appeared during the conversation but I could hear his voice). When I reached the end of that repeated segment a new one began instantly (the one that should had been presented in the first place).
The second bug happened when I reached Alistair, after killing the demons as well. When I approached him a second time the conversation went as expected, but at the end it repeated. Pretty much like what happened with Leliana, but in the last segment instead of in the first.
Also, I remember now, having another issue on this same quest, when I was in the Fade visiting the different islands and earning shapeshifting skills. I think it happened with the NPC which gives you the Golem Form, when I got to his room he was being attacked by three golems, after defeating them I initiated a conversation. The game didn't let me do it, instead it saved (as if I did encountered a checkpoint) and the NPC said "Help me..." or something like that. I had to run around the chamber a bit until the conversation was eventually triggered.
One last thing. I had some visual corruption when playing the game after installing the patch. It happened mainly during the Loading screens where it could have a strange yellow glow or black/gray layers instead of the standard presentation. Also I noted problems in the blood of fallen enemies (a square red area around the enemy) or flashing clothes. This was eventually fixed after restarting the game twice.
These aren't major bugs but still I think they are worth mentioning.
