Everybody except the crew of my ship.
I wasn't about to rush into the Omega-4 Relay without being fully prepared and once there I wasn't about to break up my team just to rescue one person.
Sorry, Dr. Chakwas.

On my first go I lost only Jack. To be honest though, I kinda wish I lost more - all the hype and then we push through it like smooth-criminals. I mean, how bad-ass is Shepard?
Answer: veeeery bad-ass

1st time: No one. (Was only playing on veteran though.)
2nd time: Haven't got there yet ....
Keep making new characters to try out and keep ignoring my FemShep infiltrator. Really should finish her game off so I can have her ready for ME3.

it still confuses me how people manage to loose team members on that mission. you do all the loyalty missions and select the most logical people for each part.

My first play I lost the guy who was going to die anyway and the machine who probably backed itself up at the last minute.
I only lost Legion because I left Zaeed in charge... and I lost Thane because he was distracted by Kolyat when I sent him to escort the crew. Martyred himself

All except poor Thane. Thought it would be a good idea to put him on the "suicide mission" as Tech specialist instead of Jacob. Thought he was made for the job since he seemed eager about it when I recruited him. I was wrong. Shot in the head. Thane was cool. Should have put Tali there cause I know now she would have lived through it, whereas it's impossible for Thane to live. Oh well live and learn.

First playthrough
1. Choose Miranda for Biotic Barrier -> Grunt dies
2. Choose Zaed as squad leader... Zaed dies at the door
3. Choose Miri instead of Jack in their quarrel, -> Jack died at holding the line
Second playthrough
Hopefully no-one... (well Samara will go down... couse I go with Morinth this time around...)

Oh gee... thats a few play throughs at this point.
Umm, I think I lost half my crew.I stopped to do an n7 or something, or maybe that was the second time around. No, had to be the second playthrough they got the quizinart treatment. I was a little disappointed, I thought I'd lose someone.

Everyone.
Not to sound conceited, but I actually find it difficult to understand how you could have anyone die during it without actually trying to get them killed on purpose. With the possible exception of Mordin during the hold the line sequence, it all seemed very, very straightforward to me, even without the use of a guide. I guess everyone sees it different.

First run, Jack went down after the final battle. Lost her loyalty in that Jack/Miranda dispute, couldn't regain it and dragged her along anyway. And I didn't feel sorry at all. Hell yes, growing up as an orphan on the streets of Earth wasn't that much fun either, lady, but I didn't turn into a homicidal maniac, did I?
This weekend will be very different with Tali not yet recruited, Grunt still in his tub and Legion still deactivated. I plan on getting back with just the abducted crew, Mordin and Garrus.

Everyone survived my first time through. Was a bit disappointed actually, as the suicide mission felt a little too easy to survive. But have since decided that if the developers had put in hidden elements that factored into the survivability of my crew, I'd be frustrated and rageful about not figuring it out. Anything less than a perfect run my first time out would have felt like the game was calling me a failure. If I'm going to fail, it's going to be on purpose.
I suppose a couple elements are a bit less intuitive, such as needing to upgrade Jack's biotics, but I never turned down the opportunity of an upgrade. They could have been cruel and put in decisions that came out of left field, such as shooting Conrad in the foot meant that Crewman Redshirt was going to die.

The Hold the Line sequence was the only part of the Suicide Run that had any element of surprise, but I didn't figure that out until going to the forums because I took Mordin with me at the very end. They could have been less obvious, such as not specifically telling you that you needed a tech specialist to go through the ducts. I'm sure a lot more people would have sent Jacob the Volunteer through that part. I mean, accidentally sent him. I'm aware many people purposefully sent him through.

1st playthrough, Mordin died, then I reloaded the whole mission and sent him off on escort duty, everyone survived from that playthrough on wards.
